Privacy Policy & Record Keeping
New Data Protection Legislation comes in to force in the UK on 25th May 2018. The General Data Protection Regulation (GDPR), is EU wide legislation, and is currently being enacted into UK law and will become the 2018 Data Protection Act. Personal data has been defined by the act as ‘any information relating to an identifiable person who can be directly or indirectly identified’, this will include such data as name and contact details, but may also information such as IP Addresses.
The personal data I collect about you will include data relating to your name, address, date of birth, wider contact details and data relating to all aspects of your health. I will process your personal data to allow me to provide you with my services as your medical herbalist in assessing your health needs and prescribing a personalised formula of herbal medicines. I will only use your data for the purpose for which it was collected. I will only share your data with other health practitioners (e.g your GP, or another herbalist) with your permission. I will only grant access to or share your data with authorised third parties where I am required or entitled to do so by law under lawful data processing.
I currently record your personal data on paper along with any notes made during a consultation and do not store any of the above information electronically. Consultation notes are kept in a locked box. It is a condition of my Insurance Policy to take and retain client records. The policy wording notes and current advice is that records shall be kept for at least 7 years following the last occasion on which treatment was given. In the case of treatment to minors, it is advisable that records should be kept or at least 7 years after they reach the age of majority (18).
